All first-time patients have to fill in a Medical History form before your appointment to cut down on the amount of time in the clinic. If you haven’t had a chance to fill out our Medical History Form before you come in, please be sure to arrive 5-10 minutes before your scheduled appointment to give you some time to fill in this form.
Be sure to check out our Pre/ Post form– some of our treatments require that you do or don’t do certain things before your treatment. If you’re not sure, or if you have any questions, give us a call.
If you can’t make your appointment, let us know as soon as possible and we’ll do our best to re-schedule you. We will charge a $20 fee for any cancellation of appointments made with less than 24 hours notice. Some treatments may require a deposit to hold the appointment.
Unfortunately, we book our treatments back to back. We really want to provide you with the best possible treatment. If you are more than 15 minutes late, we will not be able to treat you. Some late arrivals may be charged a $50 cancellation fee.
We get it. Everyone can change their mind. But unfortunately any pre-paid treatments are non refundable.
We will take back unopened, unused product up to 2 days following purchase. We are unable to take back opened or used product.

We accept the following forms of payment: Debit, Visa, MasterCard, Gift Cards.
It is critical that we have your current address, phone number and email address on file. Please make sure our front reception is made aware of any changes.
For your convenience, we utilize a text message, email reminder or phone call to confirm appointments. If appointments are not confirmed within the first 15-24 hours your appointment is canceled and fees will apply. We ask that you respect our staff by confirming appointments appropriately .
A deposit is required to book an appointment on Sunday's. ($50.00)
We require a 48 hour notice to reschedule your appointment
If you've missed your appointment, deposits are non refundable
